7	7	relating to an unsworn declaration made by an employee of a state
8	8	agency or political subdivision in the performance of the
9	9	employee's job duties.
10	10	BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS:
11	11	SECTION 1. Section 132.001, Civil Practice and Remedies
12	12	Code, is amended by amending Subsection (d) and adding Subsection
13	13	(f) to read as follows:
14	14	(d) Except as provided by Subsections [Subsection] (e) and
15	15	(f), an unsworn declaration made under this section must include a
16	16	jurat in substantially the following form:
17	17	"My name is __________ _________ ____________, my
18	18	(First) (Middle) (Last)
19	19	date of birth is _________________, and my address is
20	20	_____________, ____________, _________, ____________,
21	21	(Street) (City) (State) (Zip Code)
22	22	and __________________. I declare under penalty of
23	23	(Country)
24	24	perjury that the foregoing is true and correct.
25	25	Executed in _______ County, State of ________, on
26	26	the ________ day of ________, ________.
27	27	(Month) (Year)

29	29	Declarant"
30	30	(f) An unsworn declaration made under this section by an
31	31	employee of a state agency or a political subdivision in the
32	32	performance of the employee's job duties, must include a jurat in
33	33	substantially the following form:
34	34	"My name is ___________ __________ ___________,
35	35	(First) (Middle) (Last)
36	36	and I am an employee of the following governmental
37	37	agency: __________________. I am executing this
38	38	declaration as part of my assigned duties and
39	39	responsibilities. I declare under penalty of perjury

46	46	SECTION 2. Subsection (f), Section 132.001, Civil Practice
47	47	and Remedies Code, as added by this Act, applies only to an unsworn
48	48	declaration executed on or after the effective date of this Act. An
49	49	unsworn declaration executed before the effective date of this Act
50	50	is governed by the law in effect immediately before the effective
51	51	date of this Act, and that law is continued in effect for that
52	52	purpose.
53	53	SECTION 3. This Act takes effect September 1, 2013.
