| 11 | 11 | INFORMATION PROVIDED BY VOTER. (a) Any form | |
| 12 | 12 | completed by a voter and provided to an election officer shall | |
| 13 | 13 | include the voter's name, address, date of birth, voter unique | |
| 14 | 14 | identifier (VUID), if available, and the number from either a | |
| 15 | 15 | driver's license, election identification certificate, or personal | |
| 16 | 16 | identification card issued to the person by the Department of | |
| 17 | 17 | Public Safety or last four digits of their social security number. | |
| 18 | 18 | (b) A person desiring to register to vote who does not | |
| 19 | 19 | possess the information required by Section 13.002(c)(8)(A) or (B) | |
| 20 | 20 | may provide a statement under Section 13.002(c)(8)(C). | |
| 21 | 21 | SECTION 2. This Act takes effect January 1, 2018. |
