| 8 | 8 | SECTION 1. Chapter 1, Election Code, is amended by adding | |
| 9 | 9 | Section 1.021 to read as follows: | |
| 10 | 10 | SECTION 1.021 INFORMATION PROVIDED BY VOTER. (a)Any form | |
| 11 | 11 | completed by a voter and provided to an election officer shall | |
| 12 | 12 | include the voter's name, address, date of birth, voter unique | |
| 13 | 13 | identifier (VUID), if available, and the number from either a | |
| 14 | 14 | driver's license, election identification certificate, or personal | |
| 15 | 15 | identification card issued to the person by the Department of | |
| 16 | 16 | Public Safety or last four digits of their social security number. | |
| 17 | 17 | (b) A person desiring to register to vote who does not | |
| 18 | 18 | possess the information required by Section 13.002(c) (8)(A) or (B) | |
| 19 | 19 | may provide a statement under Section 13.002(c)(8)(C). | |
| 20 | 20 | SECTION 2. This Act takes effect January 1, 2018. |
