| 5 | 5 | | WHEREAS, Mary Nelle Cummins has greatly benefited the |

| 6 | 6 | | residents of Collin County as the founder of Community Health |
|---|
| 7 | 7 | | Clinic; and |
|---|
| 8 | 8 | | WHEREAS, Ms. Cummins established Community Health Clinic in |
|---|
| 9 | 9 | | 2004 as a nonprofit organization offering quality health care to |
|---|
| 10 | 10 | | underserved families; over the years, the clinic has gained the |
|---|
| 11 | 11 | | support of medical partners, volunteers, community members, and |
|---|
| 12 | 12 | | churches; since its opening, it has expanded its services and hours |
|---|
| 13 | 13 | | of operation, and today it provides care to approximately 750 |
|---|
| 14 | 14 | | uninsured patients in the northwest Dallas area; and |
|---|
| 15 | 15 | | WHEREAS, CHC offers a wide range of services, including |
|---|
| 16 | 16 | | pediatric care, women's wellness exams, prescription assistance, |
|---|
| 17 | 17 | | counseling, and treatment for diabetes, asthma, and other chronic |
|---|
| 18 | 18 | | conditions; and |
|---|
| 19 | 19 | | WHEREAS, Through her compassion, vision, and initiative, |
|---|
| 20 | 20 | | Mary Nelle Cummins has made a lasting and positive difference in the |
|---|
| 21 | 21 | | lives of her fellow citizens, and she may reflect with pride on her |
|---|
| 22 | 22 | | dedicated efforts to make Collin County a better place for all; now, |
|---|
| 23 | 23 | | therefore, be it |
|---|
| 24 | 24 | | RESOLVED, That the House of Representatives of the 88th Texas |
|---|
| 25 | 25 | | Legislature hereby honor Mary Nelle Cummins as the founder of |
|---|
| 26 | 26 | | Community Health Clinic of Collin County and extend to her sincere |
|---|
| 27 | 27 | | best wishes for continued success with her important work; and, be |
|---|
