14	14	To amend title XVIII of the Social Security Act to require
15	15	hospitals to provide information on available hospice pro-
16	16	grams to certain individuals upon discharge under the
17	17	Medicare program.
18	18	Be it enacted by the Senate and House of Representa-1
19	19	tives of the United States of America in Congress assembled, 2
20	20	SECTION 1. SHORT TITLE. 3
21	21	This Act may be cited as the ‘‘End-of-life Access to 4
22	22	Supportive and Essential care Act of 2025’’ or the ‘‘EASE 5
23	23	Act of 2025’’. 6

31	31	(a) I
32	32	NGENERAL.—Section 1861(ee)(2)(D) of the So-5
33	33	cial Security Act (42 U.S.C. 1395x(ee)(2)(D)) is amend-6
34	34	ed— 7
35	35	(1) by inserting ‘‘, home health services,’’ after 8
36	36	‘‘including hospice care’’; 9
37	37	(2) by striking ‘‘including the availability of 10
38	38	home health services through individuals and enti-11
39	39	ties’’ and inserting the following: ‘‘including— 12
40	40	‘‘(i) in the case of individuals who are like-13
41	41	ly to need home health services, the availability 14
42	42	of such services through home health agencies’’; 15
43	43	(3) by striking ‘‘listed by the hospital as avail-16
44	44	able and, in the case of individuals who are likely to 17
45	45	need post-hospital extended care services,’’ and in-18
46	46	serting the following: ‘‘listed by the hospital as avail-19
47	47	able; 20
48	48	‘‘(ii) in the case of individuals who are 21
49	49	likely to need post-hospital extended care serv-22
50	50	ices,’’; 23
51	51	(4) by striking the period and inserting ‘‘; 24
52	52	and’’; and 25

58	58	‘‘(iii) in the case of individuals who are 3
59	59	likely eligible for hospice care, the availability of 4
60	60	such care through hospice programs that par-5
61	61	ticipate in the program under this title and that 6
62	62	serve the area in which the patient resides.’’. 7

64	64	FFECTIVEDATE.—The amendments made by 8
65	65	paragraph (1) shall apply with respect to discharges occur-9
66	66	ring on or after January 1, 2026. 10
