1212 Statement of purpose of bill as introduced: This bill proposes to establish a 7
1313 two-year pilot for one or more community nurse programs serving aging 8

1717 It is hereby enacted by the General Assembly of the State of Vermont: 12
1818 Sec. 1. COMMUNITY NURSE PROGRAM PILOT SERVING AGING 13

2020 (a) $450,000.00 shall be appropriated to the Department of Disabilities, 15
2121 Aging, and Independent Living for the purpose of establishing a two-year pilot 16
2222 to enhance the provision of services by one or more existing community nurse 17
2323 programs to aging Vermonters in the program’s region. 18
2424 (b) The Department shall seek and consider applications for participation in 19
2525 the pilot described in this section from existing community nurse programs 20 BILL AS INTRODUCED S.33

3030 serving aging Vermonters in the program’s region. The Department shall 1
3131 select one or more programs, as funding permits, to participate in the pilot 2
3232 based on a program’s application materials. A successful application shall 3
3333 include a plan to: 4
3434 (1) create a community health worker position to serve program clients; 5
3535 (2) expand the program’s nursing services to serve a greater percentage 6
3636 of aging Vermonters in the program’s region, including ensuring that nurse 7
3737 home visits are not limited in number or duration; 8
3838 (3) identify and fill the program’s coverage gaps; 9
3939 (4) integrate enhanced administrative support within the program; and 10
4040 (5) develop a system to obtain relevant data and document program 11
4141 needs, outcomes, and processes. 12
4242 (c) On or before December 15, 2027, a program participating in the pilot 13
4343 described in this section shall submit a report to the Department detailing the 14
4444 number of aging Vermonters served, program gaps, recommended program 15
4545 improvements, and any other data relevant to the pilot. 16
4646 Sec. 2. EFFECTIVE DATE 17
4747 This act shall take effect on July 1, 2025. 18
