| 18 | 18 | | Sec. 1. COMMUNITY NURSE PROGRAM PILOT SERVING AGING 13 |

| 20 | 20 | | (a) $450,000.00 shall be appropriated to the Department of Disabilities, 15 |
|---|
| 21 | 21 | | Aging, and Independent Living for the purpose of establishing a two-year pilot 16 |
|---|
| 22 | 22 | | to enhance the provision of services by one or more existing community nurse 17 |
|---|
| 23 | 23 | | programs to aging Vermonters in the program’s region. 18 |
|---|
| 24 | 24 | | (b) The Department shall seek and consider applications for participation in 19 |
|---|

| 30 | 30 | | serving aging Vermonters in the program’s region. The Department shall 1 |
|---|
| 31 | 31 | | select one or more programs, as funding permits, to participate in the pilot 2 |
|---|
| 32 | 32 | | based on a program’s application materials. A successful application shall 3 |
|---|
| 33 | 33 | | include a plan to: 4 |
|---|
| 34 | 34 | | (1) create a community health worker position to serve program clients; 5 |
|---|
| 35 | 35 | | (2) expand the program’s nursing services to serve a greater percentage 6 |
|---|
| 36 | 36 | | of aging Vermonters in the program’s region, including ensuring that nurse 7 |
|---|
| 37 | 37 | | home visits are not limited in number or duration; 8 |
|---|
| 38 | 38 | | (3) identify and fill the program’s coverage gaps; 9 |
|---|
| 39 | 39 | | (4) integrate enhanced administrative support within the program; and 10 |
|---|
| 40 | 40 | | (5) develop a system to obtain relevant data and document program 11 |
|---|
| 41 | 41 | | needs, outcomes, and processes. 12 |
|---|
| 42 | 42 | | (c) On or before December 15, 2027, a program participating in the pilot 13 |
|---|
| 43 | 43 | | described in this section shall submit a report to the Department detailing the 14 |
|---|
| 44 | 44 | | number of aging Vermonters served, program gaps, recommended program 15 |
|---|
| 45 | 45 | | improvements, and any other data relevant to the pilot. 16 |
|---|
| 46 | 46 | | Sec. 2. EFFECTIVE DATE 17 |
|---|
| 47 | 47 | | This act shall take effect on July 1, 2025. 18 |
