| 19 | 19 | | Be it enacted by the Senate and House of Representatives in General |
|---|
| 20 | 20 | | Assembly convened: |

| 22 | 22 | | Section 1. (Effective July 1, 2019) Not later than October 1, 2019, the 1 |
|---|
| 23 | 23 | | Department of Public Health shall develop and make available a form 2 |
|---|
| 24 | 24 | | to serve as the statement to be (1) presented, in accordance with the 3 |
|---|
| 25 | 25 | | provisions of section 10-204a of the general statutes, by the parents or 4 |
|---|
| 26 | 26 | | guardians of a child that an immunization required pursuant to said 5 |
|---|
| 27 | 27 | | section is contrary to the religious beliefs of such child or the parents 6 |
|---|
| 28 | 28 | | or guardians of such child, and (2) acknowledged, in accordance with 7 |
|---|
| 29 | 29 | | the provisions of sections 1-32, 1-34 and 1-35 of the general statutes, by 8 |
|---|
| 30 | 30 | | (A) a judge of a court of record or a family support magistrate, (B) a 9 |
|---|
| 31 | 31 | | clerk or deputy clerk of a court having a seal, (C) a town clerk, (D) a 10 |
|---|
| 32 | 32 | | notary public, (E) a justice of the peace, (F) an attorney admitted to the 11 |
|---|
| 33 | 33 | | bar of this state, or (G) notwithstanding any provision of chapter 6 of 12 |
|---|
| 34 | 34 | | the general statutes, a school nurse. The department shall include a 13 |
|---|
| 35 | 35 | | notice on such form that any person authorized to acknowledge such 14 |
|---|
| 36 | 36 | | statement pursuant to section 10-204a of the general statutes has the 15 |
|---|
| 37 | 37 | | discretion to acknowledge or refuse to acknowledge such statement. 16 |
|---|

| 50 | 50 | | the statement of a parent or guardian of a child that immunization is 23 |

| 51 | 51 | | contrary to the religious beliefs of such child or the parent or guardian 24 |
|---|
| 52 | 52 | | of such child, pursuant to section 10-204a of the general statutes, has 25 |
|---|
| 53 | 53 | | the discretion to acknowledge or refuse to acknowledge such 26 |
|---|
| 54 | 54 | | statement. Each local or regional board of education shall distribute 27 |
|---|
| 55 | 55 | | such written notification to each administrator, as defined in section 28 |
|---|
| 56 | 56 | | 10-144e of the general statutes, employed by such board. 29 |
