| 24 | 24 | | Public Health to submit an annual report regarding adverse reactions to 2 |

| 25 | 25 | | immunizations, which shall include, but need not be limited to, (1) the 3 |

| 26 | 26 | | total number of adverse reactions reported during the previous calendar 4 |
|---|
| 27 | 27 | | year, (2) reported adverse reactions, disaggregated by immunization 5 |
|---|
| 28 | 28 | | type, lot number and age of recipient, (3) the total number and cost of 6 |
|---|
| 29 | 29 | | emergency department visits related to the reported adverse reactions, 7 |
|---|
| 30 | 30 | | (4) the total number of hospitalizations, permanent disabilities and 8 |

| 31 | 31 | | deaths related to the reported adverse reactions, and (5) a summary of 9 |

| 32 | 32 | | activities undertaken by the Department of Public Health to support the 10 |
