| 26 | 26 | diabetes or insulin-using diabetes, and (2) requiring review, appeal or 4 | |
| 27 | 27 | approval of any such insulin drug prior to a medical provider 5 | |
| 28 | 28 | prescribing such insulin drug to such covered person. 6 | |
