Be It Enacted by the Legislature of the State of Florida: 9

Section 1. Section 607.0122, Florida Statutes, is amended 11
to read: 12
607.0122 Fees for filing documents and issuing 13
certificates.— 14
(1) The department shall collect the following fees when 15
the documents described in this section are delivered to the 16
department for filing: 17
(a)(1) Articles of incorporation: $35. 18
(b)(2) Application for registered name: $87.50. 19
(c)(3) Application for renewal of registered name: $87.50. 20
(d)(4) Corporation's statement of change of registered 21
agent or registered office or both if not included on the annual 22
report: $35. 23
(e)(5) Designation of and acceptance by registered agent: 24
$35. 25

(f)(6) Agent's statement of resignation from a corporation 26

(2) A person may request expedited services by submitting 57
to the department an expedited service request form. A request 58
for expedited services must be made in person or through an 59
electronic transmission. A request for expedited services may 60
not be requested through mail. 61
Section 2. Subsection (12) of section 610. 104, Florida 62
Statutes, is amended to read: 63
610.104 State authorization to provide cable or video 64
service.— 65
(12) Beginning 5 years after approval of the 66
certificateholder's initial certificate of franchise issued by 67
the department, and every 5 years the reafter, the 68
certificateholder shall update the information contained in the 69
original application for a certificate of franchise. At the time 70
of filing the information update, the certificateholder shall 71
pay a processing fee of $1,000. Any certificateholde r that fails 72
to file the updated information and pay the processing fee on 73
the 5-year anniversary dates shall be subject to cancellation of 74
its state-issued certificate of franchise authority if, upon 75

notice given to the certificateholder at its last addre ss on 76
file with the department, the certificateholder fails to file 77
the updated information and pay the processing fee within 30 78
days after the date notice was mailed. The application and 79
processing fees imposed in this section shall be paid to the 80
Department of State for deposit into the Clearing Funds Trust 81
Fund for immediate transfer by the Chief Financial Officer to 82
the General Inspection Trust Fund of the Department of 83
Agriculture and Consumer Services. The Department of Agriculture 84
and Consumer Services shall maintain a separate account within 85
the General Inspection Trust Fund to distinguish cable franchise 86
revenues from all other funds. The application, any amendments 87
to the certificate, or information updates must be accompanied 88
by a fee to the Depar tment of State equal to that for filing 89
articles of incorporation pursuant to s. 607.0122(1)(a) s. 90

Section 3. This act shall take effect upon becoming a law 92
if HB 1365 or similar legislation is adopted in the same 93
legislative session or an extension thereof and becomes law . 94
