| 14 | 14 | | 4 Section 5. The College Student Immunization Act is amended |

| 15 | 15 | | 5 by changing Section 6 as follows: |

| 17 | 17 | | 7 Sec. 6. Report. Within 8 weeks after the the commencement |

| 18 | 18 | | 8 of classroom instruction, the post-secondary educational |

| 19 | 19 | | 9 institution shall file a report with the Department stating |
|---|
| 20 | 20 | | 10 the number of persons attending the institution who had |
|---|
| 21 | 21 | | 11 presented: proof of immunization; certification of medical |
|---|
| 22 | 22 | | 12 exemption; statement of religious objection; and no proof of |
|---|
| 23 | 23 | | 13 immunization. The report shall be on forms prepared by the |
|---|
| 24 | 24 | | 14 Department. |
