11	11	1 AN ACT concerning criminal law.
12	12	2 Be it enacted by the People of the State of Illinois,
13	13	3 represented in the General Assembly:
14	14	4 Section 5. The Sex Offender Registration Act is amended by
15	15	5 changing Section 4 as follows:
16	16	6 (730 ILCS 150/4) (from Ch. 38, par. 224)
17	17	7 Sec. 4. Discharge of sex offender, as defined in Section 2
18	18	8 of this Act, or sexual predator from Department of Corrections
19	19	9 facility or other penal institution; duties of official in
20	20	10 charge. Any sex offender, as defined in Section 2 of this Act,
21	21	11 or sexual predator, as defined by this Article, who is
22	22	12 discharged, paroled or released from a Department of
23	23	13 Corrections or Department of Juvenile Justice facility, a
24	24	14 facility where such person was placed by the Department of
25	25	15 Corrections or Department of Juvenile Justice or another penal
26	26	16 institution, and whose liability for registration has not
27	27	17 terminated under Section 7 shall, prior to discharge, parole
28	28	18 or release from the facility or institution, be informed of
29	29	19 his or her duty to register in person within 3 days of release
30	30	20 by the facility or institution in which he or she was confined.
31	31	21 The facility or institution shall also inform any person who
32	32	22 must register that if he or she establishes a residence
33	33	23 outside of the State of Illinois, is employed outside of the

68	68	1 State of Illinois, or attends school outside of the State of
69	69	2 Illinois, he or she must register in the new state within 3
70	70	3 days after establishing the residence, beginning employment,
71	71	4 or beginning school.
72	72	5 The facility shall require the person to read and sign
73	73	6 such form as may be required by the Illinois State Police
74	74	7 stating that the duty to register and the procedure for
75	75	8 registration has been explained to him or her and that he or
76	76	9 she understands the duty to register and the procedure for
77	77	10 registration. The facility shall further advise the person in
78	78	11 writing that the failure to register or other violation of
79	79	12 this Article shall result in revocation of parole, aftercare
80	80	13 release, mandatory supervised release or conditional release.
81	81	14 The facility shall obtain information about where the person
82	82	15 expects to reside, work, and attend school upon his or her
83	83	16 discharge, parole or release and shall report the information
84	84	17 to the Illinois State Police. The facility shall give one copy
85	85	18 of the form to the person and shall send one copy to each of
86	86	19 the law enforcement agencies having jurisdiction where the
87	87	20 person expects to reside, work, and attend school upon his or
88	88	21 her discharge, parole or release and retain one copy for the
89	89	22 files. Electronic data files which includes all notification
90	90	23 form information and photographs of sex offenders being
91	91	24 released from an Illinois Department of Corrections or
92	92	25 Illinois Department of Juvenile Justice facility will be
93	93	26 shared on a regular basis as determined between the Illinois
