4-AN ACT concerning civil law.
5-Be it enacted by the People of the State of Illinois,
6-represented in the General Assembly:
7-Section 5. The Freedom of Information Act is amended by
8-changing Section 7.5 as follows:

17-(b) Library circulation and order records identifying
18-library users with specific materials under the Library
19-Records Confidentiality Act.
20-(c) Applications, related documents, and medical
21-records received by the Experimental Organ Transplantation
22-Procedures Board and any and all documents or other
23-records prepared by the Experimental Organ Transplantation
24-Procedures Board or its staff relating to applications it
25-has received.
26-(d) Information and records held by the Department of

73-(n) Defense budgets and petitions for certification of
74-compensation and expenses for court appointed trial
75-counsel as provided under Sections 10 and 15 of the
76-Capital Crimes Litigation Act (repealed). This subsection
77-(n) shall apply until the conclusion of the trial of the
78-case, even if the prosecution chooses not to pursue the
79-death penalty prior to trial or sentencing.
80-(o) Information that is prohibited from being
81-disclosed under Section 4 of the Illinois Health and
82-Hazardous Substances Registry Act.
83-(p) Security portions of system safety program plans,

586-(ii) other reliable proof, as determined by the
587-Department, of the person's identity and age.

590-The Illinois Department shall provide information to all
591-families, orally by an intake worker and in writing when the
592-application is filed, about the availability and location of
593-immunization services.
594-(5) Once an applicant is determined eligible for aid, he
595-or she has the right to request to have the case transferred to
596-another local office of the Department of Human Services for
597-his or her convenience based on one of the following factors:
598-the location of his or her employer; the location of his or her
599-child care provider; access to reliable transportation; or the
600-location of a social service provider that he or she sees on a
601-regular basis. Within 5 business days after the request for
602-transfer, the Department shall transfer the case, assign a
603-caseworker, make appropriate entries in the computer system,
604-and issue a written notice to the recipient that includes the
605-name of and contact information for the caseworker. The
606-location of the recipient's case may be reconsidered on the
607-recipient's request or at the time of redetermination of

677-(a) Any party aggrieved by conduct or regulation in

682-federal claim. Any action brought under this Act shall be

655+11 Sec. 11-15. Application requirements.
656+12 (1) An application for financial aid shall be filed in
657+13 writing by the person requesting aid and, in the case of a

659+15 otherwise permitted in paragraph (2). Applications for aid
660+16 under Articles III, IV, and V shall be filed in writing with
661+17 any local office of the Department of Human Services in the
662+18 manner prescribed by the Department. Applications for aid

734+20 (4) If an application for financial aid is filed for a
735+21 family, and any person in that family is under 18 years of age,
736+22 the application shall be accompanied by the following for each
737+23 such person under 18 years of age:
738+24 (i) a copy of the person's birth certificate, or
739+25 (ii) other reliable proof, as determined by the
740+26 Department, of the person's identity and age.

752+2 families, orally by an intake worker and in writing when the
753+3 application is filed, about the availability and location of
754+4 immunization services.
755+5 (5) Once an applicant is determined eligible for aid, he
756+6 or she has the right to request to have the case transferred to
757+7 another local office of the Department of Human Services for
758+8 his or her convenience based on one of the following factors:
759+9 the location of his or her employer; the location of his or her
760+10 child care provider; access to reliable transportation; or the
761+11 location of a social service provider that he or she sees on a
762+12 regular basis. Within 5 business days after the request for
763+13 transfer, the Department shall transfer the case, assign a
764+14 caseworker, make appropriate entries in the computer system,
765+15 and issue a written notice to the recipient that includes the
766+16 name of and contact information for the caseworker. The
767+17 location of the recipient's case may be reconsidered on the
768+18 recipient's request or at the time of redetermination of
769+19 eligibility.

790+6 (2) the treatment of gender dysphoria or the
791+7 affirmation of an individual's gender identity or gender
792+8 expression, including, but not limited to, all supplies,
793+9 care, and services of a medical, behavioral health, mental
794+10 health, surgical, psychiatric, therapeutic, diagnostic,
795+11 preventative, rehabilitative, or supportive nature that is
796+12 not unlawful under the laws of this State, including on
797+13 any theory of vicarious, joint, several, or conspiracy
798+14 liability.
799+15 "Lawful health care activity" means seeking, providing,
800+16 receiving, assisting in seeking, providing, or receiving,
801+17 providing material support for, or traveling to obtain lawful
802+18 health care.
803+19 "Health records related to lawful health care" means
804+20 records that identify a person who has sought or received
805+21 lawful healthcare, including, but not limited to, information
806+22 regarding an individual's medical history, mental or physical
807+23 condition, or medical treatment or diagnosis by a healthcare
808+24 professional, and insurance and billing records for medical
809+25 care.
810+26 "Location information related to lawful health care" means

861+6 Sec. 28-12. Cause of action.
862+7 (a) Any party aggrieved by conduct or regulation in
863+8 violation of Section 28-11 of this Act may bring a civil action
864+9 in a federal district court or State circuit court against the
865+10 offending unit of government. Any State claim brought in
866+11 federal district court shall be a supplemental claim to a
867+12 federal claim. Any action brought under this Act shall be
868+13 commenced within 2 years after the cause of action was
869+14 discovered.
870+15 (b) Upon motion, a court shall award reasonable attorney's
871+16 fees, court costs, and litigation expenses, including expert
872+17 witness fees and other litigation expenses, to a plaintiff who
873+18 is a prevailing party in any action brought under this Act. In
874+19 awarding reasonable attorney's fees, the court shall consider
875+20 the degree to which the relief obtained relates to the relief
876+21 sought.

900+11 Article VII of the Illinois Constitution.
