44 2 WHEREAS, The members of the Illinois Senate are saddened
55 3 to learn of the death of Jerome "Jerry" Slowik, who passed away
66 4 on October 2, 2022 at the age of 75; and
77 5 WHEREAS, Jerry Slowik was born to Stanley and Irene Slowik
88 6 on August 25, 1947; he lived in both River Forest and Elmwood
99 7 Park; and
1010 8 WHEREAS, Jerry Slowik graduated from St. Patrick High
1111 9 School; he earned his Bachelor of Science and his Master of
1212 10 Science in Taxation from DePaul University; and
1313 11 WHEREAS, Jerry Slowik worked as an accountant and
1414 12 controller for several firms, the Pizzerias Uno and Due, and
1515 13 El Milagro; and
1616 14 WHEREAS, Jerry Slowik served as treasurer of the
1717 15 Nineteenth Century Charitable Association; and
1818 16 WHEREAS, Jerry Slowik participated in a singles tennis
1919 17 club and was a fan of the Chicago White Sox, the Chicago
2020 18 Blackhawks, and Northwestern University; and
2121 19 WHEREAS, Jerry Slowik was preceded in death by his

3131 2 WHEREAS, At the time of his passing, Jerry Slowik was
3232 3 survived by his wife, Helene (Bak) Slowik; his son, Joseph
3333 4 "Joe" Slowik; his cousin, Maryann Tolzien; his
3434 5 brothers-in-law, Martin, Richard, and Bernie; and his nephews;
3535 6 therefore, be it
3636 7 RESOLVED, BY THE SENATE OF THE ONE HUNDRED THIRD GENERAL
3737 8 ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of
3838 9 Jerome "Jerry" Slowik and extend our sincere condolences to
3939 10 his family, friends, and all who knew and loved him; and be it

4141 12 RESOLVED, That a suitable copy of this resolution be
