| 5 | 5 | | 3 to learn of the death of Jerome "Jerry" Slowik, who passed away |

| 6 | 6 | | 4 on October 2, 2022 at the age of 75; and |

| 7 | 7 | | 5 WHEREAS, Jerry Slowik was born to Stanley and Irene Slowik |

| 8 | 8 | | 6 on August 25, 1947; he lived in both River Forest and Elmwood |
|---|
| 9 | 9 | | 7 Park; and |
|---|
| 10 | 10 | | 8 WHEREAS, Jerry Slowik graduated from St. Patrick High |
|---|
| 11 | 11 | | 9 School; he earned his Bachelor of Science and his Master of |
|---|
| 12 | 12 | | 10 Science in Taxation from DePaul University; and |
|---|
| 13 | 13 | | 11 WHEREAS, Jerry Slowik worked as an accountant and |

| 14 | 14 | | 12 controller for several firms, the Pizzerias Uno and Due, and |
|---|
| 15 | 15 | | 13 El Milagro; and |
|---|
| 16 | 16 | | 14 WHEREAS, Jerry Slowik served as treasurer of the |

| 17 | 17 | | 15 Nineteenth Century Charitable Association; and |
|---|
| 18 | 18 | | 16 WHEREAS, Jerry Slowik participated in a singles tennis |

| 19 | 19 | | 17 club and was a fan of the Chicago White Sox, the Chicago |

| 20 | 20 | | 18 Blackhawks, and Northwestern University; and |

| 21 | 21 | | 19 WHEREAS, Jerry Slowik was preceded in death by his |
