 2025 and 2026 HB3589 Introduced , by Rep. Nabeela Syed SYNOPSIS AS INTRODUCED: 105 ILCS 5/22-80.5 new Amends the General Provisions Article of the School Code. Provides that students in grades 9 through 12 must satisfactorily pass a medical evaluation each year and, if applicable, receive an electrocardiogram before participating in an interscholastic athletic activity. Creates a waiver exception. Provides that a student in any grade who participates in an interscholastic athletic activity or who is a candidate for an interscholastic athletic activity's team must satisfactorily pass a medical evaluation each year before participating in the interscholastic athletic activity or engaging in any practice, tryout, workout, conditioning, or other physical activity associated with the student's candidacy for the interscholastic athletic activity. Requires that a medical evaluation only be administered by a licensed health care professional or a physician. Provides requirements for the content of a preparticipation physical evaluation form. 1  AN ACT concerning education.
2  Be it enacted by the People of the State of Illinois,
3  represented in the General Assembly:
4  Section 5. The School Code is amended by adding Section
5  22-80.5 as follows:
6  (105 ILCS 5/22-80.5 new)
7  Sec. 22-80.5. Student athletes; medical evaluation and
8  electrocardiogram.
9  (a) Definitions. In this Section:
10  "Athletic trainer" means an athletic trainer licensed
11  under the Illinois Athletic Trainers Practice Act who is
12  working under the supervision of a physician.
13  "Coach" means any volunteer or employee of a school who is
14  responsible for organizing and supervising students to teach
15  them or train them in the fundamental skills of an
16  interscholastic athletic activity. "Coach" refers to both head
17  coaches and assistant coaches.
18  "Interscholastic athletic activity" means any organized
19  school-sponsored or school-sanctioned activity for students,
20  generally outside of school instructional hours, under the
21  direction of a coach, athletic director, or band leader,
22  including, but not limited to, baseball, basketball,
23  cheerleading, cross country, fencing, field hockey, football,

1  golf, gymnastics, ice hockey, lacrosse, marching band, rugby,
2  soccer, skating, softball, swimming and diving, tennis, track
3  (indoor and outdoor), ultimate frisbee, volleyball, water
4  polo, and wrestling. All interscholastic athletics are deemed
5  to be interscholastic activities.
6  "Licensed health care professional" means a person who is
7  a nurse, a physical therapist licensed under the Illinois
8  Physical Therapy Act, an occupational therapist licensed under
9  the Illinois Occupational Therapy Practice Act, a physician
10  assistant, or an athletic trainer.
11  "Nurse" means a person who is employed by or volunteers at
12  a school and is licensed under the Nurse Practice Act as a
13  registered nurse, practical nurse, or advanced practice
14  registered nurse.
15  "Physician" means a physician licensed to practice
16  medicine in all of its branches under the Medical Practice Act
17  of 1987.
18  "Physician assistant" means a physician assistant licensed
19  under the Physician Assistant Practice Act of 1987.
20  "School" means any public or private elementary or
21  secondary school, including a charter school.
22  "Student" means an adolescent or child enrolled in a
23  school.
24  (b) Students in grades 9 through 12 must satisfactorily
25  pass a medical evaluation each year and, if applicable,
26  receive an electrocardiogram before participating in an

1  interscholastic athletic activity. The electrocardiogram may
2  be waived if a parent or guardian of the student objects to the
3  procedure based on religious tenets or practices or submits a
4  written request to waive the requirement.
5  A student in any grade who participates in an
6  interscholastic athletic activity or who is a candidate for an
7  interscholastic athletic activity's team must satisfactorily
8  pass a medical evaluation each year before participating in
9  the interscholastic athletic activity or engaging in any
10  practice, tryout, workout, conditioning, or other physical
11  activity associated with the student's candidacy for the
12  interscholastic athletic activity, including activities that
13  occur outside of the school year. The medical evaluation may
14  be administered only by a licensed health care professional or
15  a physician.
16  (c) After a medical evaluation of a student, the licensed
17  health care professional or the physician performing the
18  evaluation must complete a preparticipation physical
19  evaluation form. The evaluation form must incorporate the
20  recommendations of the American Heart Association for
21  participation in cardiovascular screening and include all of
22  the following:
23  (1) a place for the signature of the licensed health
24  care professional or the physician performing the
25  evaluation with an attestation that each examination
26  procedure listed on the form was performed by the licensed

1  health care professional or the physician or by someone
2  under the direct supervision of the licensed health care
3  professional or the physician;
4  (2) a place for the licensed health care professional
5  or the physician to indicate if a referral to another
6  practitioner was made instead of the completion of a
7  certain examination procedure;
8  (3) if applicable, a place for the practitioner to
9  whom the student was referred to complete the remaining
10  sections and attest to that portion of the examination;
11  and
12  (4) a section advising students to complete a
13  cardiovascular assessment and informing students of
14  information concerning alternative cardiovascular
15  evaluation and diagnostic tests.
16  (d) Results of a medical evaluation of a student must be
17  provided to the student's school.
