| 7 | 7 | | 1.7 Subd. 9.Authorization of services by waiver case managers.(a) This subdivision |
|---|
| 8 | 8 | | 1.8applies to authorizations for housing stabilization services provided to an individual eligible |
|---|
| 9 | 9 | | 1.9for waiver services under chapter 256S or section 256B.092 or 256B.49. By signing a |
|---|
| 10 | 10 | | 1.10person-centered plan, an eligible provider agrees to provide the services described in the |
|---|
| 11 | 11 | | 1.11person-centered plan. After submission of a person-centered plan signed by an eligible |
|---|
| 12 | 12 | | 1.12provider to the individual's waiver case manager, the individual's waiver case manager shall |
|---|
| 13 | 13 | | 1.13determine if the individual is eligible for housing stabilization services and approve housing |
|---|
| 14 | 14 | | 1.14stabilization services subject to the following limits: |
|---|
| 15 | 15 | | 1.15 (1) up to 150 hours of housing transition services per transition, no more than half of |
|---|
| 16 | 16 | | 1.16which may be provided remotely; |
|---|
| 17 | 17 | | 1.17 (2) up to 150 hours of housing sustaining services annually, no more than half of which |
|---|
| 18 | 18 | | 1.18may be provided remotely; |
|---|
| 19 | 19 | | 1.19 (3) up to $3,000 for moving expenses per eligibility span; and |
|---|
| 20 | 20 | | 1.20 (4) housing consultation services once annually. |

| 21 | 21 | | 1.21 (b) The individual's waiver case manager must submit any request for exceptions to the |
|---|
| 22 | 22 | | 1.22service limits described in paragraph (a) to the commissioner of human services for approval. |
