12 (2)Thedateofthemarriageandtheplaceatwhichitisregistered; 13 (3)Thedateonwhichthepartiesseparated; 14 (4)Thename,age,andaddressofeachchild,andtheparentwithwhomeachchild 15hasprimarilyresidedforthesixtydaysimmediatelyprecedingthefilingofthepetitionfor 16dissolutionofmarriageorlegalseparation; 17 (5)Whetherthewifeispregnant; however,pregnancystatusshallnotpreventthe 18courtfromenteringajudgmentofdissolutionofmarriageorlegalseparation; 19 (6)ThelastfourdigitsoftheSocialSecuritynumberofthepetitioner,respondentand 20eachchild; 21 (7)Anyarrangementsastothecustodyandsupportofthechildrenandthe 22maintenanceofeachparty;and 23 (8)Thereliefsought. 24 3.Uponthefilingofthepetitioninaproceedingfordissolutionofmarriageorlegal 25separation,eachchildshallimmediatelybesubjecttothejurisdictionofthecourtinwhichthe 26proceedingiscommenced,unlessaproceedinginvolvingallegationsofabuseorneglectof 27thechildispendinginjuvenilecourt.Untilpermittedbyorderofthecourt,neitherparent 28shallremoveanychildfromthejurisdictionofthecourtorfromanyparentwithwhomthe HB280 2 29childhasprimarilyresidedforthesixtydaysimmediatelyprecedingthefilingofapetitionfor 30dissolutionofmarriageorlegalseparation. 31 4.Themerefactthatoneparenthasactualpossessionofthechildatthetimeoffiling 32shallnotcreateapreferenceinfavorofsuchparentinanyjudicialdeterminationregarding 33custodyofthechild. 34 5.Therespondentshallbeservedinthemannerprovidedbytherulesofthesupreme 35courtandapplicablecourtrulesand,toavoidaninterlocutoryjudgmentofdefault,shallfilea 36verifiedanswerwithinthirtydaysofthedateofservicewhichshallnotonlyadmitordeny 37theallegationsofthepetition,butshallalsosetforth: 38 (1)ThelastfourdigitsoftheSocialSecuritynumberofthepetitioner,respondentand 39eachchild;
