5948 BE IT ENACTED BY THE PEOPLE OF THE STAT E OF OKLAHOMA:
6049 SECTION 1. AMENDATORY 26 O.S. 2011, Section 5 -111, as
6150 last amended by Section 1, Chapter 402, O.S.L. 2013 (26 O.S. Supp.
6251 2020, Section 5-111), is amended to read as foll ows:
6352 Section 5-111. A. Forms to be used for filing Declarations of
6453 Candidacy shall be prescribed by the Secretary of the State Election
6554 Board and shall contain the following information:
6655 1. The name of the candidate as it shall a ppear on the ballot;
6756 2. The legal name of the candida te;
57+3. The candidate's place of residence and mailing address;
58+4. The name of the office sought;
59+5. The candidate's date of birth;
60+6.The a. the current political party affiliation of the
61+candidate seeking political party nomination as shown

95-3. The candidate's place of residence and mailing address;
96-4. The name of the office sought;
97-5. The candidate's date of birth;
98-6.The a. the current political party affiliation of the
99-candidate seeking political party nomination as shown
10088 on the candidate's current voter re gistration record,

10290 b. the date the candidate registered as a voter of that

10492 registration, pursuant to subsection D of Section 4 -
10593 110.1 of this title, and
10694 c. if the date shown in subparagraph b of this p aragraph
10795 is six (6) months or less immediately preceding the
10896 first day of the filing period but the candidate was
10997 previously registered in a different county prior to
11098 registering in the current county, the candidate shall
11199 provide the name of the previou s county and the date

115103 7. The voter registration identification number of the
116104 candidate and the precinct and county wherein the candidate is a

145-8. An oath wherein the candidate swears or affirms that he or
146-she is qualified to become a candidate for the office sought, and
147-that, if elected, the candidate will be qualified to hold the
148-office; and
149-9. Any additional information which the Secretary deems
150-necessary.
151138 B. If the candidate has ever been ultimate ly determined by a
152139 court of proper authority to be guilty of an offense specified in
153140 subsection A of Section 5 -105a of this title or at the time o f
154141 filing the Declaration of Candidacy is named in an outstanding
155142 warrant for arrest for such an offense, in th is or any other state,
156143 the candidate shall provide the following information on a form
157144 prescribed by the Secretary of the State Election Board:
158145 1. The name of the offense;
159146 2. The date of conviction or issuance of the outstanding
160147 warrant; and
161148 3. The county and state of conviction or issuance of the
162149 outstanding warrant.
163150 The provisions of this subsection shall not app ly to an offense
164151 for which the candidate has received a pardon.
165152 C. A Declaration of Candidacy form must be signed by the
166153 candidate, and the s ignature must be properly nota rized by a notary
167154 public or other person authorized by law to administer oaths.
155+D. In addition to the information r equired by this section, a
156+candidate may include a telephone number, email address and website
157+address. Such additional information shall n ot be required of any
158+candidate but if provided shall be made available to the publi c.
159+SECTION 2. This act shall become effective November 1, 2021.
