48	39		office; amending 26 O.S. 2011, Section 5-111, as last
49	40		amended by Section 1, Chapter 402, O.S.L. 2013 (26
50	41		O.S. Supp. 2020, Section 5 -111), which relates to
51	42		declaration of candid acy forms; adding information to
52	43		be included on declaration of candidacy form ; and

59	48		BE IT ENACTED BY THE PEOPLE OF THE STAT E OF OKLAHOMA:
60	49		SECTION 1. AMENDATORY 26 O.S. 2011, Section 5 -111, as
61	50		last amended by Section 1, Chapter 402, O.S.L. 2013 (26 O.S. Supp.
62	51		2020, Section 5-111), is amended to read as foll ows:
63	52		Section 5-111. A. Forms to be used for filing Declarations of
64	53		Candidacy shall be prescribed by the Secretary of the State Election
65	54		Board and shall contain the following information:
66	55		1. The name of the candidate as it shall a ppear on the ballot;
67	56		2. The legal name of the candida te;
	57	+	3. The candidate's place of residence and mailing address;
	58	+	4. The name of the office sought;
	59	+	5. The candidate's date of birth;
	60	+	6.The a. the current political party affiliation of the
	61	+	candidate seeking political party nomination as shown

104	92		registration, pursuant to subsection D of Section 4 -
105	93		110.1 of this title, and
106	94		c. if the date shown in subparagraph b of this p aragraph
107	95		is six (6) months or less immediately preceding the
108	96		first day of the filing period but the candidate was
109	97		previously registered in a different county prior to
110	98		registering in the current county, the candidate shall
111	99		provide the name of the previou s county and the date
112	100		the candidate registered as a voter of that political
113	101		party in the previous county pursuant to subsection D
114	102		of Section 4-110.1 of this title;
115	103		7. The voter registration identification number of the
116	104		candidate and the precinct and county wherein the candidate is a
117	105		registered voter;
	106	+	8. An oath wherein the candidate swears or affirms that h e or
	107	+	she is qualified to becom e a candidate for the office sought, and
	108	+	that, if elected, the candidate will be qua lified to hold the
	109	+	office; and
	110	+	9. Any additional information which the Secretary deems

145		-	8. An oath wherein the candidate swears or affirms that he or
146		-	she is qualified to become a candidate for the office sought, and
147		-	that, if elected, the candidate will be qualified to hold the
148		-	office; and
149		-	9. Any additional information which the Secretary deems

151	138		B. If the candidate has ever been ultimate ly determined by a
152	139		court of proper authority to be guilty of an offense specified in
153	140		subsection A of Section 5 -105a of this title or at the time o f
154	141		filing the Declaration of Candidacy is named in an outstanding
155	142		warrant for arrest for such an offense, in th is or any other state,
156	143		the candidate shall provide the following information on a form
157	144		prescribed by the Secretary of the State Election Board:
158	145		1. The name of the offense;
159	146		2. The date of conviction or issuance of the outstanding
160	147		warrant; and
161	148		3. The county and state of conviction or issuance of the
162	149		outstanding warrant.
163	150		The provisions of this subsection shall not app ly to an offense
164	151		for which the candidate has received a pardon.
165	152		C. A Declaration of Candidacy form must be signed by the
166	153		candidate, and the s ignature must be properly nota rized by a notary
167	154		public or other person authorized by law to administer oaths.
	155	+	D. In addition to the information r equired by this section, a
	156	+	candidate may include a telephone number, email address and website
	157	+	address. Such additional information shall n ot be required of any
	158	+	candidate but if provided shall be made available to the publi c.
	159	+	SECTION 2. This act shall become effective November 1, 2021.
