33172 BE IT ENACTED BY THE PEOPLE OF THE STATE OF OKLAHOMA:

35173 SECTION 1. AMENDATORY 26 O.S. 2021, Section 5 -111, is
36174 amended to read as follows:

38175 Section 5-111. A. Forms to be used for filing Declarations of
39176 Candidacy shall be prescri bed by the Secretary of the Sta te Election
40177 Board and shall contain the following information:

42178 1. The name of the candidate as it shall a ppear on the ballot;

44179 2. The legal name of the candi date;

73223 B. The address of residence and the mailing address of a
74224 candidate provided on a Declaration of Can didacy shall be verified
75225 by the secretary of the election board with whom the Declaration was
76226 filed and shall be kept confidential; provided, however, such
77227 address information shall be provided to a candidate , candidate
78228 representative, or other lawful authori ty in anticipation of or as
79229 part of a contest of can didacy or contest of an election as provided
80230 for in this title or as part of a petition challenge as provided by
81231 law.

83258 C. 1. Prior to filing a Declaration of Candidacy, a candidate
84259 shall obtain a voter registration verification form from the
85260 secretary of the election board with whom the candidate is required
86261 to file. The candidate shall provide the form with the Declaration
87262 of Candidacy.

89263 2. A voter registration verification form shall include:

93264 a. the candidate’s name, date of birth, address of
94265 residence, party affiliation, voter identification
95266 number, the precinct and county where the candidate is
96267 registered, and the original date of voter
97268 registration within the county, if available, and

99269 b. the districts in which th e candidate is registered to
100270 vote including but not limited to state senator and
101271 representative, county commissioner, sch ool and
102272 technology center, and municipality. If applicable, a
103273 ward or member district shall be included for the
104274 school or technology center district or municipality.

106275 3. The secretary of the election board with whom the candidate
107276 is required to file shall only accept a Declaration of Candidacy if
108277 the information contained on the voter registration verification
109278 form matches the information provided on the Declaration of
110279 Candidacy, and if such Declaration meets all other requirements of
111280 state law.

113307 4. The provisions o f this subsection shall not apply to
114308 candidates for federal office.

116309 5. The Secretary of the State Election Board shall prescribe
117310 the voter registration verification form required by this
118311 subsection.

120312 D. If the candidate has ever been ultimately determined by a
121313 court of proper authority to be guilty of an offense specified in
122314 subsection A of Section 5 -105a of this title or at the time of
123315 filing the Declaration of Candidacy is named in an outstanding
124316 warrant for arrest for such an offense, in this or any oth er state,
125317 the candidate shall provide the follo wing information on a form
126318 prescribed by the Secretary of the State Election Board:

128319 1. The name of the offense;

130320 2. The date of conviction or issuance of the outstanding
131321 warrant; and

138324 The provisions of this subsection shall not app ly to an offense
139325 for which the candidate has received a pardon.

141326 C. E. A Declaration of Candidacy form must be signed by the
142327 candidate, and the signature m ust be properly notarized by a notary
143328 public or other person authorized by law to administer oaths.
