188 | 94 | | the latest Federal Decennial Census. The pilot program shall be |

189 | 95 | | supported by employees of the Department of Corrections focusing |
---|
190 | 96 | | primarily on case ma nagement services for eligible inmates selected |
---|
191 | 97 | | for the pilot program. Any inmate with a medium or high risk of |
---|
192 | 98 | | felony arrest shall be eligible for p articipation in the pilot |
---|
193 | 99 | | program. |
---|
194 | 100 | | C. Employees of the Departm ent shall assist inmates on parole |
---|
195 | 101 | | who are assigned to the pilot program in managing basic needs, |
---|
196 | 102 | | including housing, job training and placement, medical and mental |
---|
197 | 103 | | health care and any additional programming or responsibilities |
---|
198 | 104 | | attendant to the terms of the reentry requirements of the inmate. |
---|
199 | 105 | | Employees of the Department shall also work closely with the inmates |
---|
200 | 106 | | to prepare, monitor, revise, and fulfill individualized inmate |
---|
201 | 107 | | reentry plans consiste nt with this section during the ter m of the |
---|
202 | 108 | | pilot program. |

203 | 109 | | D. Individualized inmate reentry plans shall fo cus on |

204 | 110 | | connecting inmates to services for which the inmate is eligible |

205 | 111 | | under existing federal, state, and local laws. |

294 | 202 | | 14. Transitional support services, including transportation |

295 | 203 | | services, twenty-four-hour on-call support and conflict resolutio n |

296 | 204 | | support. |
---|
297 | 205 | | F. The Department shall be auth orized to enter into public- |
---|
298 | 206 | | private partnerships to assist in providing higher education |
---|
299 | 207 | | programs and reentry services for inmates and funding said program s |
---|
300 | 208 | | and services for the pilot program. |
---|
301 | 209 | | G. The Department s hall be authorized to accept funding for th e |
---|
302 | 210 | | pilot program from philanthropic organizations and federal grants. |
---|
303 | 211 | | H. The Department shall contract for an evaluation of the pilot |
---|
304 | 212 | | program that will asse ss its effectiveness in reducing recidivism |
---|
305 | 213 | | among offenders transitioning from prison into the commu nity. |
