7	7	relating to an unsworn declaration made by an employee of a state
8	8	agency or political subdivision or a private process server in the
9	9	performance of the employee's or server's job duties.
10	10	BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS:
11	11	SECTION 1. Section 132.001, Civil Practice and Remedies
12	12	Code, is amended by amending Subsection (d) and adding Subsections
13	13	(f) and (g) to read as follows:
14	14	(d) Except as provided by Subsections [Subsection] (e),
15	15	(f), and (g), an unsworn declaration made under this section must
16	16	include a jurat in substantially the following form:
17	17	"My name is __________ _________ ____________, my
18	18	(First) (Middle) (Last)
19	19	date of birth is _________________, and my address is
20	20	_____________, ____________, _________, _________,
21	21	(Street) (City) (State) (Zip Code)
22	22	and __________________. I declare under penalty of
23	23	(Country)
24	24	perjury that the foregoing is true and correct.
25	25	Executed in _______ County, State of ________, on
26	26	the ________ day of________, ________.

30	30	(f) An unsworn declaration made under this section by an
31	31	employee of a state agency or a political subdivision in the
32	32	performance of the employee's job duties, must include a jurat in
33	33	substantially the following form:

36	36	and I am an employee of the following governmental
37	37	agency: __________________. I am executing this

62	62	SECTION 2. Sections 132.001(f) and (g), Civil Practice and
63	63	Remedies Code, as added by this Act, apply only to an unsworn
64	64	declaration executed on or after the effective date of this Act. An
65	65	unsworn declaration executed before the effective date of this Act
66	66	is governed by the law in effect immediately before the effective
67	67	date of this Act, and that law is continued in effect for that
68	68	purpose.
69	69	SECTION 3. This Act takes effect September 1, 2013.
