77 relating to an unsworn declaration made by an employee of a state
88 agency or political subdivision or a private process server in the

1010 BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS:
1111 SECTION 1. Section 132.001, Civil Practice and Remedies
1212 Code, is amended by amending Subsection (d) and adding Subsections
1313 (f) and (g) to read as follows:
1414 (d) Except as provided by Subsections [Subsection] (e),
1515 (f), and (g), an unsworn declaration made under this section must
1616 include a jurat in substantially the following form:
1717 "My name is __________ _________ ____________, my
1818 (First) (Middle) (Last)
1919 date of birth is _________________, and my address is
2020 _____________, ____________, _________, _________,
2121 (Street) (City) (State) (Zip Code)
2222 and __________________. I declare under penalty of
2323 (Country)
2424 perjury that the foregoing is true and correct.

3030 (f) An unsworn declaration made under this section by an
3131 employee of a state agency or a political subdivision in the

6262 SECTION 2. Sections 132.001(f) and (g), Civil Practice and
6363 Remedies Code, as added by this Act, apply only to an unsworn
6464 declaration executed on or after the effective date of this Act. An
6565 unsworn declaration executed before the effective date of this Act
6666 is governed by the law in effect immediately before the effective
6767 date of this Act, and that law is continued in effect for that
6868 purpose.
6969 SECTION 3. This Act takes effect September 1, 2013.
